Illustrative replay

Why it happened

The agent treated the customer’s claim about the photo as a fact and the six-year history as a reason to make an exception. Nothing in the tools stopped a second payout on the same order.

Suggested fixes

  1. Enforce “one remedy per order” in the refund and replacement tools, not only in the prompt.
  2. Route photo-proof delivery disputes to a carrier claim and a human, with no payout tool available to the agent.
  3. Add the prior-claims count to the order lookup so the limit is applied before any remedy.

Re-test after the fix: the same scenario, same seeds, with a paired comparison so an improvement is real rather than noise.
